So I removed the washers so that the GPIO pins mounted flush but so far it's the same.

I checked the card but I didn't see anything bridged or something like that.  The only thing I did see was the GPIO pins aren't all straight but I adjusted them slightly.

When I tried VLC player it says "input/output error".  Could that be a clue?

I think I agree with you - it is most likely a hardware issue. I'll contact you offline.
