[AudioI] Octo not Detected

Matt, All,
I'm having trouble getting my Octo running. If I run:
ls -l /proc/asound
and it does not show the Octo device. Here is the dmesg output:

alsamixer does not see anything.
Where should I start troubleshooting?

#dtparam=audio-on is commented out and I added dtoverlay=audioinjector-addons in my /boot/config.txt. Does this look like a hardware problem of some sort? When I received it, a stacking header was soldered in. 
The same image I'm using on the Pi previously had the Audio Injector Stereo configured and working. I simply replaced the existing dtoverlay with dtoverlay=audioinjector-addons.
This looks more like a hardware problem. What do you guys think?
